- If an evaluation is warranted and agreed upon, 50% of the total is due in order to schedule the first testing session.
- The remaining balance is due before the feedback session can be scheduled.
- All rating scales/paperwork must be returned in order for the report to be completed.
- Once paid in full and all paperwork has been completed, the report will be available within 30 days.
- Jade Bloom Psychoed Services LLC is considered an "out of network" provider and does not bill insurance companies. However, we can provide a Superbill upon request to help you seek reimbursement.
- Many insurance companies require prior authorizations and/or additional documentation before testing can be conducted.
- A Superbill will include essential information for your insurance provider such as CPT codes, NPI numbers, EIN, and diagnosis codes. It is important to reach out to your insurance provider for any questions regarding coverage.
- The cost of an evaluation depends on the type of evaluation and the referral concern. The estimated cost of evaluations range from $500 to $2000 with an average cost between $800-$1800.
- The cost of a formal intake appointment is $150 (subtracted from the total cost of the evaluation).
- Before a testing session is scheduled, a Good Faith Estimate will be provided.

How long do evaluations take?
Psychoeducational evaluations typically take 3 to 5 hours of direct testing, depending on the client's age, attention, pace, and the type of evaluation needed. Some assessments may be completed in one session, while others may be split across multiple sessions to ensure the client performs at their best.
In addition to testing time, the examiner also spends several hours gathering additional data from school or medical providers (if applicable), parents/caregivers, reviewing records (if applicable), scoring, analyzing results, and preparing the written report.
